I live in FL (Polk County) and would like my girlfriend to drive my car for Uber, exactly what is required insurance wise for the state of FL, to do this?

Sorry for the delay in responding, Marc. This one slipped through the cracks.
First of all, if your girlfriend would like to drive for Uber, she would also need to sign up and have an account with Uber. She is not allowed to simply take over your shift, impersonating you.
Assuming she is an approved driver, she would have to register your car with Uber. For this to happen, they will ask for her name to be attached to the insurance. This means that you would have to add her to your auto policy. Every state has different rules about car insurance, so you may want to ask your insurance company about that. (say in Massachusetts, you can add "authorized" users, who are usually family members but not necessarily have to be.)
