You can do that with the APP on your phone.  You simply download the app and then schedule the trip or wait until you're ready and then request the ride.  I would recommend getting a Uber XL if that is available so there will be plenty of room for the wheelchair.  You can also call the driver once they accept the trip and make sure they're comfortable transporting the individual, although I'm confident they will be.  I've picked up several people that were in a chair, helped them into the car, and then stored their chair in the back until arriving.
