Ok since a few people have wondered about this:
How much does it cost to charge a scooter? Less than you might think. Just to run some rough numbers:
The scooter batteries' capacities are reported to be about 8800mAh at 36V. We can convert this to watt hours: 8800 * 36 / 1000 = 316.8Wh
Electricity costs on average of 12 cents per kilowatt hour (ie. 1000 watt hours) in the US. So, the cost of charging one of these scooters from empty is going to be something like:
(316.8 / 1000) * 12 = 4 cents
It's probably a bit more than that since chargers are not 100% efficient. And, the cost of electricity varies between different locations, so take this with a grain of salt. But it's safe to say the electricity cost of charging these things is not something to worry much about.

You can book an Uber online through their website. However, your best bet might be to just order a traditional taxi.
Yes you can. However with uber it's just going to make the ride request for you in time for 4am. You are not reserving a driver.
With Lyft they'll actually let drivers commit to the ride ahead of time. So I'd lean toward doing this with Lyft rather than Uber.
I think an Uber X will do. Depending on how large the box is... If it'll fit in a sedan somewhere you should be good.

On Android there are location faker apps that theoretically will allow you to fake your phone into being wherever you want. I haven't tried them so I can't say whether this will work for sure. I don't know if iOS has similar apps.

A lot of times they do, and it can sometimes be at least partially covered by insurance/medicare/medicaid. In those cases transportation is provided by local providers with good drivers and vehicles.

Hi, yes you could book the ride ahead. Then at the appointed time, the Uber app will tell you the driver is on the way and what car to look for.
However you will probably also be fine just opening the app and booking when you're ready to leave the airport, without booking ahead. That way you don't have to worry about getting the timing right. In any reasonably populated area, Ubers around the airport should be plentiful.
