What happens in many cases is the driver arrives at the address the passenger has entered as their pick-up location, but there is no passenger present. Once he arrives at waits for more than a minute, if he starts moving (say he just realized there was an error in your pick up address) or for whatever reason he starts moving... the APP will sometimes assume the driver "forgot" to start the trip and will do it for you. I've had that happen a few times and even as a driver it is VERY annoying as we have to go through extra steps to cancel the trip and furthermore don't get the cancellation fee for a "no show"... remember we never wait beyond 5 minutes.
This doesn't explain the "driver doesn't pick up my phone" part of the question.
but it's a good insight. This is a sort of thing riders have no idea about.
I am still waiting for that option. This happens WAY TOO OFTEN.

This is pretty common - has definitely happen to me a couple times before. I don't get why Uber is not cracking down harder on this issue.
It isn't always deliberate. Riders may try to steal other people's Ubers for a free ride. (They call it Uberjacking - https://ride.guru/lounge/p/5-rider-scams-that-game-the-system-drivers-watch-out)
