A 45 miles will take 45~60 minutes to drive. Such a trip should be about $55 to 70 in most areas. This is without considering for surge pricing which can raise the fare during busy periods.

That's correct! Uber's rider app no longer shows the surge pricing multiplier - in all regions. This has been replaced by "upfront pricing" as Uber claims that it eliminates confusion and encourages users to keep using their apps.
So they figured people rather not know when the price is surging. They just prefer knowing what the price will be.
They removed the lightning bolts for the surge prices as well. Great point.
For your trips out to work, it's likely that the demand is higher. The price fluctuates due to Uber and Lyft's new pricing methods. (See exception under the Guru answer)
Uber and Lyft do not always disclose "surges" or "high-demand" pricing to the users anymore, so they quote you a price for your particular route and particular time. I would imagine the demand on the way to work is higher for you.
The generic public often has to be at work between 8 and 9AM, but there is a larger window for getting off work, say between 3 and 6. That may attribute to the difference.
Yes, you would totally be accepted for UberBlack (if the car is literally black) or UberSelect. I wouldn't worry for a second. (It's luxurious, it has 4-doors and takes 4 passengers.) If you already have the car just submit your application, and they should approve you for it. (After signing up as a driver and it doesn't automatically enable the UberSelect option, make sure to contact Uber!

Yes, UberX has expanded technically to the entire state of New Hampshire, meaning the service is available anywhere as long as there is driver availability.
For example, try running an estimate on RideGuru from Rochester to Dover.
Rochester is near UNH, right? I am pretty sure there are drivers around!

Good call about the scheduled rides. Yes, Lyft drivers can see the destination and accept them if there are requests in the scheduled queue.
